Personalized Analgesic Therapy: Targeting Pain at Its Source | On-demand Version (December 2022)

Course Description

Chronic pain is the primary reason patients seek healthcare, affecting more than 1.5 billion people worldwide, and, despite advances in pain research and knowledge, pain remains inadequately treated. Challenges in current pain management can be described by:

  1. Adverse systemic side effects, including GI ulcers, liver toxicity, CNS effects, and opioid use disorder
  2. Inadequate therapeutic efficacy (30-50% of individuals with chronic pain report pain is not adequately controlled)
  3. Poor patient compliance resulting from adverse side effects and inadequate therapeutic outcomes
  4. The need for greater understanding by the medical community on how to appropriately target pain pathways

The overarching objective of this program is to address the 4 major challenges described above:

  • Reduce systemic adverse events through topical and local analgesic applications
  • Optimize therapeutic outcomes by carefully targeting pain at its source, selecting appropriate analgesics based on pharmacodynamics, and applying appropriate combination therapy
  • Improve patient compliance by reducing adverse events, improving outcomes, and simplifying treatment regimens
  • Provide healthcare providers the tools to appropriately assess and classify pain, identify anatomical structures and pathways, and appropriately utilize analgesics based on pharmacodynamics, safety, and efficacy profiles – ultimately reducing adverse events, improving outcomes, and reducing opioid use

At this 2-day event, day 1 will be dedicated to laying the foundations for an alternative and improved approach to pain management by reviewing pain physiology (types of pain, pain pathways & neural systems, and sensitization), analgesic pharmacology of 40+ ingredients (pharmacodynamics, efficacy, safety, and dosing), and topical therapy applications (skin physiology, delivery systems, and formulation considerations). The skills and knowledge that is acquired from day 1 will be put to work on day 2 with patient case assessments and formula investigations for common pain or pain-related conditions, including osteoarthritis, low back pain, diabetic neuropathy, anal fissures, fibromyalgia, surgical pain, and opioid use disorder.

Learning Objectives

Pharmacists

  1. Identify challenges in pain management including adverse systemic reactions, inadequate therapeutic outcomes, poor patient compliance, and the need for education in pain management.
  2. Classify different types of pain, notably nociceptive pain, neuropathic pain, functional pain, and mixed pain.
  3. Investigate the neural systems and sensitization processes involved at each level of the pain pathway, including the periphery, the dorsal horn, the ascending pathway, the brain, and the descending pathway.
  4. Evaluate and organize current evidence on analgesic pharmacology with special focus on pharmacodynamics, efficacy, safety, and dosing of over 40 analgesics.
  5. Utilize fundamental concepts of topical therapy, including skin anatomy (target layers), physiology (lipids, hydration, temperature, pH, metabolism, and microbiome), individual differences (age, ethnicity, and sensitivities), and formulation consideration (delivery systems, premade base concepts, penetration enhancers, among more).
  6. Apply skills and knowledge in the form of case assessments and formula investigations for common pain or pain-related conditions.

Pharmacy Technicians

  1. Identify challenges in pain management including adverse systemic reactions, inadequate therapeutic outcomes, poor patient compliance, and the need for education in pain management.
  2. Classify different types of pain, notably nociceptive pain, neuropathic pain, functional pain, and mixed pain.
  3. Investigate analgesic targets for pain management.
  4. Evaluate and organize current evidence on analgesic pharmacology with special focus on pharmacodynamics, efficacy, safety, and dosing of over 40 analgesics.

Describe important variables for topical therapy application, including skin anatomy (target layers), physiology (lipids, hydration, temperature, pH, metabolism, and microbiome), individual differences (age, ethnicity, and sensitivities), and formulation consideration (delivery systems, premade base concepts, penetration enhancers, among more).
  6. Assess different case scenarios and critically evaluate formulation treatment options for different patients.

Dr. de la Garza is a board certified physician who practices multidisciplinary interventional pain management in four offices in the Tampa Bay area. He performs interventional pain procedures, specializing in minimally invasive surgical decompression of herniated discs and spinal stenosis, implantable intrathecal pumps, spinal cord stimulation, and peripheral nerve stimulation. Dr. de la Garza has published numerous research papers and abstracts, and has presented at regional and national academic conferences on the topics of anesthesia and pain medicine. He is also a consultant for numerous pharmaceutical and medical device manufacturers.

Dr. de la Garza received a Bachelor of Science in Molecular Biology and a Bachelor of Arts in Chemistry from Texas Lutheran College in Seguin, Texas. He graduated from the University of Texas Southwestern Medical School in 2001 and performed his anesthesia residency at the University of South Florida College of Medicine, Department of Anesthesiology and Critical Care from 2001 to 2005. He then completed an interventional pain medicine fellowship at The Cleveland Clinic, in the Department of Anesthesia and Pain Management.

Dr. Ken Speidel is well recognized for his broad experience in pharmacy practice and education with pharmacotherapeutic specialties in hormone therapy and pain management. Dr. Speidel is a leading subject-matter expert in pharmaceutical compounding with expertise in sterile and non-sterile compounding processes for the acute care, community, outpatient, and 503A/503B practice sectors. Dr. Speidel has played instrumental roles in the development of national standards for pharmacy compounding practices and provides consulting services to many organizations, including boards of pharmacy and hospital and health systems in the United States and abroad.

In addition to his worldwide consulting and educational work, Dr. Speidel is a retired Professor of Pharmacy Practice from the University of Findlay. Over the years, Dr. Speidel has assisted in the development and facilitation of many nationally recognized educational programs, approved by the Accreditation Council for Pharmacy Education (ACPE). Dr. Speidel served as senior surveyor and advisor for the development of the Pharmacy Compounding Accreditation Board (PCAB). Dr. Speidel has served as multi-term president of the National Home Infusion Association (NHIA), president of Hospice of Portage, and has been awarded fellowship status with the Alliance for Pharmacy Compounding (APC) and the American College of Apothecaries (ACA). Dr. Speidel received a Bachelor of Science and Doctor of Pharmacy from Ohio Northern University and has completed postdoctoral training in nutritional support and functional medicine.
