Overview
This session focuses on how pharmacists can support younger patients navigating hormonal changes, with a primary emphasis on perimenopause. We’ll cover symptom recognition, lab interpretation, and thoughtful hormone therapy strategies, along with a brief look at related conditions like PMDD and PMOS. The goal is to give clinicians practical, real-world guidance they can use to better support this often overlooked patient population.

Pharmacists
- Recognize common symptoms and hormonal changes associated with perimenopause and related conditions affecting younger female patients
- Discuss the pharmacist’s role in supporting patients with hormone-related concerns through symptom assessment, patient education, and collaborative care
- Review general treatment considerations for perimenopause, including lifestyle strategies and hormone therapy approaches
- Describe key clinical features of related hormone conditions such as PMDD and PMOS.
Pharmacy Technicians
- Identify common symptoms and concerns associated with perimenopause and other hormone-related conditions that may be encountered in the pharmacy setting
- Describe how pharmacy technicians can support patients receiving hormone-related therapies through effective communication, workflow coordination, and patient engagement

Tana Kaefer is the Director of Clinical Services at Bremo Pharmacies in Richmond, Virginia, where she leads innovative, patient-centered clinical programs focused on expanding access to care. She earned her Doctor of Pharmacy from Virginia Commonwealth University School of Pharmacy and completed a PGY-1 community-based pharmacy residency at Bremo, which shaped her career in advancing pharmacist-led clinical services.
Dr. Kaefer specializes in hormone consulting, with a focus on perimenopause, menopause, and bioidentical hormone therapy, and works closely with patients and providers to deliver individualized, evidence-informed care. She also serves as a Clinical Assistant Professor at VCU, where she precepts pharmacy students and residents.
A recognized leader in community pharmacy, Dr. Kaefer has been instrumental in advancing immunization services and improving vaccination rates through innovative, pharmacy-led care models and public health partnerships. She continues to drive practice transformation through her work with CPESN Virginia. She was named the 2025 Outstanding Pharmacist in Virginia by the Virginia Pharmacists Association and is a frequent speaker on women’s health, immunizations, and advancing clinical services in community pharmacy.
